Empty soul gems can also be harvested from geode veins, which are only found in Blackreach. Each mined vein will result in 2–3 soul gems of any size, including black soul gems. The mined soul gems are leveldependant; at level 16 grand and black soul gems will start to appear, and the probability for either will max out at level 31.

There are about 30 filled soul gems available for the taking in the College of Winterhold (check both the ArchMage''s Quarters and all the dorm rooms). Once you take them all, simply wait two weeks (takes about ten minutes real time) and then all of the gems will have respawned. For me, this has been the easiest way to get soul gems yet.

I think Soul Gems are just naturally occuring gems which happen to be well suited to holding souls. Other vessels can hold souls, like the Soul Sword of Prince A''tor, but Soul Gems just seem to do it best. We know where they come from deep underground. In Blackreach there are ''geode'' veins, where you can mine soul gems.

Beside that, it seems that the consensus is that they are mined from the ground around Blackreach and Solstheim, where magical ores (Aethernium, Ebony and Heart stone) are found. You can mine soul gems at geode veins( You may find grand and black soul gems around level 31) There are 14 Corundum veins, 6 ebony veins, 34 geode veins, 3 gold veins, 2 moonstone veins, 3 orchalcum ore veins, 3 quicksilver veins, and 3 silver veins. If you look closely, there is another dwemer ruin called silent ruins.
There are 36 geode veins scattered throughout Blackreach: 34 in the cavern itself, and 2 in Silent City Catacombs. 23 of these geode veins can be mined to yield leveldependent empty soul gems (grand soul gems and black soul gems start to appear at level 16 with a very low probability, that increases till level 30 (grand)/31 (black)).
